In the matter of Part 6 of the Pharmacy Act 2007

Mr John (Sean) Oakes MPSI (Reg. No. 5886)

Following the submission of a report of the Professional Conduct Committee (the “Committee”), the Council, at its meeting on 20 June 2019, decided to attach conditions to his registration as a pharmacist. 

The conditions, as set out below, have expired. However, in accordance with the PSI’s Publications & Disclosure Policy 2022, public notice of the conditions which attached to the registration of a pharmacist or pharmacy following fitness to practise proceedings shall remain on the website for a period of 2 years following the expiry of those conditions. 

Conditions formerly attached to Mr Oakes’ registration as a pharmacist are as follows: 

(i) That Mr Oakes shall within four weeks of these conditions coming into effect enter into a formal mentoring programme with a senior and experienced superintendent pharmacist to be approved by the Registrar of the PSI. The terms of the mentoring programme shall be in writing and be agreed by Mr Oakes, the mentor and the Registrar of the PSI. Mr Oakes shall seek advice from and maintain regular contact with the mentor. Mr Oakes shall comply with all advice, guidance and recommendations of the mentor. Such advice may include attendance and completion of academic or vocational courses, or any other such learning and development activities as required. 

(ii) That Mr Oakes will arrange to have regular consultations with the mentor and meet with him or her as required. In the first twelve months there will be face to face meetings with the mentor at eight-week intervals at a minimum and reschedule thereafter at intervals to be agreed by the mentor with the Registrar/Council; Council decision: 20 June 2019; High Court Confirmation: 14 October 2019 Date of this Notice: 31 August 2023 

(iii) That Mr Oakes will agree to the mentor issuing reports to the Registrar/Council, which reports will be furnished at regular intervals or at the request of the Registrar; 

(iv) That Mr Oakes will agree to the mentor issuing a final report to the Registrar/Council at the conclusion of the period specified in condition e) below. In the event that the mentor is satisfied that Mr Oakes has successfully addressed the deficits in Mr Oakes’ professional practice before that date the mentor may issue the final report at such earlier time; 

(v) That this formal mentoring relationship will continue for a minimum period of eighteen months and thereafter until such time as Mr Oakes is informed by the Council that it is satisfied that Mr Oakes has successfully and adequately addressed the deficits in Mr Oakes’ professional practice. Either Mr Oakes or the Registrar may bring any application to the Council in this regard on notice to the other; and 

(vi) That Mr Oakes shall discharge any and all costs associated with the implementation of these conditions and shall recompense the PSI any costs that it may incur in enforcing these conditions.
