Restoring your Registration
If wish to return to practice as a pharmacist or pharmaceutical assistant, and you have been previously registered with the PSI, you will need to apply to us to have your name restored to the register. Completed application forms must be returned to the PSI by post.
Pharmacists
COVID-19 Emergency Restoration Process
In line with the Emergency Measures in the Public Interest (COVID-19) Act 2020, a new restoration route has been introduced to the Pharmacy Act, referred to as a Section 77 registration. This enables former PSI registrants, including pharmacists and pharmaceutical assistants, who either voluntarily withdrew from the register or were removed due to non-payment of registration fees, to have their registration restored. Previous registrants will find details about the process, the application form and guidance here. There is no fee for Section 77 applications. Successful applications will be placed on the Register for a temporary period only.
Restoration following voluntary cancellation
Pharmacists who have voluntarily cancelled their registration have the right to apply for restoration to the Register of Pharmacists, subject to the successful completion of a restoration process.

An applicant must:
- complete the restoration form, providing details of practice and confirmation of on-going CPD since cancellation from PSI Register,
- pay the specified fee,
- provide 2 recent photographs,
- provide a certificate of current professional status if registered as a pharmacist elsewhere,
- provide a criminal clearance certificate or statutory declaration, confirming professional status if not registered or practising elsewhere during the period of cancellation.

- provide a certificate of current professional status if registered as a pharmacist elsewhere,

- undertake to complete a self-assessment against the Core Competency framework to assess competency and learning needs,
- undertake to engage in a period of practical training/re-familiarisation of not less than 2 weeks if s/he has not practiced in another jurisdiction during the period of cancellation from the Register. Exemption from this requirement may be obtained:

- provide 2 recent photographs,
- provide a certificate of current professional status if registered as a pharmacist elsewhere,
- provide a criminal clearance certificate or statutory declaration confirming professional status if not registered or practising elsewhere during the period of cancellation,
- undertake to complete a self-assessment against the Core Competency framework to assess competency and learning needs,
- undertake to engage in a period of practical training/re-familiarisation of not less than 4 weeks if s/he has not practiced in another jurisdiction during the period of cancellation from the Register. Exemption from this requirement may be obtained:

Restoration following involuntary cancellation
Pharmacists
who have been involuntarily cancelled from the Register of Pharmacists
for non-payment of fees can undergo a restoration process if this
request is received by the PSI within six months following the due date
of payment of the annual fee. Until further notice please send your completed application form and fees form by email to registration@psi.ie
Re-Registration
Pharmacists
who have been involuntarily cancelled from the Register of Pharmacists
for non-payment of fees and who fail to make an application for
restoration within the six month period following the due date of
payment of the annual fee for continued registration, will only be
entitled to obtain registration as a pharmacist on the same basis as a
person seeking registration for the first time.
