Involuntary Cancellations

In accordance with the procedures prescribed under the Pharmacy Act 2007, the names of the persons listed below were, on the date specified, cancelled and removed from the Register of Pharmaceutical Assistants kept under section 13(1) of the Act, for failure to make application for continued registration as a pharmaceutical assistant in compliance with Rule 12 of the Pharmaceutical Society of Ireland (Registration) Rules 2008 (S.I. No. 494 of 2008), and for failure to pay the prescribed fee.
