If you are not happy with the treatment you have received in a pharmacy, or you have a concern about a pharmacist's conduct or behaviour, you may make a complaint to the PSI, if the treatment, conduct or behaviour is something over which we regulate. Pharmacists can order a controlled drug, destruction or duty register by filling out this online form. Orders must be placed by submitting a completed form before the 15th of the month. All orders received will be processed and delivered within 7-10 working days.
